By booking an appointment you are agreeing to:

• I consent to communicating electronically with and receiving virtual care by a physician at Medi Laos International Hospital
• I understand that personal information I provide will be used for the purposes of registration, appointment scheduling, and the provision of healthcare
• If required, I consent to allowing the Virtual Urgent Care physician to send documents (ex. prescriptions, letters) to the non-secure email address I provide during registration
• If my condition deteriorates ahead of my scheduled appointment, I understand I should go to my nearest Emergency Department for in-person care.
